Waiver Request
Signing the waiver is a two-step process.
- Fill out the form below to request a waiver. (The waiver will be emailed to you.)
- Click the link in the email and sign the document digitally.
Notice
Every player and coach must sign a waiver. For children under 18, their parents must sign the waiver.
Rink Closed
Due to the weather, we've closed the rink until the Fall.
Adult Waiver
For players 18 and over.
Child Waiver
For players under the age of 18.
